Retinal Consent
The retinal consent form explains the purpose of retinal imaging or examination, how the procedure works, and any potential risks or benefits. It confirms that you understand the process and give permission for the provider to perform the exam, helping ensure your eye health is thoroughly evaluated.
Release of Vision Records
The release of vision records form allows you to authorize our office to share your eye care records with another healthcare provider, insurance company, or third party. By signing, you give permission for your vision information to be securely transferred while protecting your privacy.
